New Patient Registration

 

You will not be registered with a named Doctor but registered with the Practice, you may choose to see any of the Doctors/Nurse at the Surgery.

New patients moving into the area may register by visiting the surgery. We have a large catchment area, we accept patients from Markyate, Flamstead, Studham, Redbourn, Dunstable, Caddington,Slip End, Gaddesden Row and Kensworth.

The receptionist will provide a new patient questionnaire and registration form. Please return  the completed form to the reception, along  with proof of identification, which could include a driving licence or a passport and a utility bill of your current  place of residence, without which you will not be able to register.

We would recommend that patients aged 75 years and  over should annually be checked by the nurse.

All New Patients who are registered at Markyate Surgery will be offered a full Health Check  that will be undertaken by  one of the Nurses or the HealthCare Assistant.
